Bezalel Care Management offers a range of services to support individuals with disabilities and the elderly, as well as their families. Our services include Care Management which involves coordinating all aspects of care for your loved one including medical care, personal care and social support. We also offer Aging in Place Services which allow your loved ones to stay in their own home while receiving the care that they need.

Additionally, we provide Family Support Services to help families navigate the challenges of caring for a loved one with a disability or age-related need. We understand that taking care of a loved one can be overwhelming and stressful and we are here to support you and guide you.
NHTD Service Provider
The NHTD (Nursing Home Transition and Diversion Medicaid Waiver Program) program is meant to assist elderly people with disabilities to manage their everyday life. It helps to stay in the community, get all benefits of independent living and eliminate risks to their health. All services provided are funded by Medicaid.
TBI Waiver
Provider
The New York State Department of Health (DOH) Traumatic Brain Injury (TBI) waiver program provides services to persons with a TBI. The purpose of the program is to help persons with a TBI live in the community setting of their choice. Medicaid funds the program.
Private Care Coordination
We're the Service Coordination Agency that provides primary assistance to the waiver participant in gaining access to the waiver and Medicaid State Plan services. Bezalel Care Management also assists with other local state and federally funded educational, vocational, social and medical services.
